I agree by checking the box below that I am signing a release hereby giving Surrey Tennis Center (the "Centre")permission to use any Pictures taken of me or my family members within the Centre for any purpose which may include among others, advertising, promotion & marketing for any program or service the Centre offers.
I agree that I have no rights to the Pictures or Content that the picture becomes part of, and all rights to the Pictures & Content belong to the Centre.
I represent and warrant that I am at least 18 years of age and have the full legal capacity to execute this release.

I agree that pictures taken of me or my family inside the club or at any club event, can be used or disseminated by the club in their materials made available to the general public.
I do not agree that pictures of me or my family can be used as described above.
